HWGYF FE 1452 Generals Youth Football, Inc. - Chapter 6, 172H CORI REQUEST FORM Hamilton-Wenham Generals Youth Football is requesting all the available Criminal Offender Record Information (CORI) on all individuals who are volunteering to coach from the Criminal History Systems Board pursuant to Chapter 6, 172H. The Criminal History Systems Board mandates, that all organizations primarily engaged in providing activities or programs to children 18 years of age or less, that accept volunteers, must obtain all CORI, regarding volunteers, prior to accepting any person as a volunteer. As an agency providing a Youth Athletic Program in the state of Massachusetts, Generals Youth Football may receive CORI pertaining to conviction and pending criminal case data for the purpose of screening current or otherwise qualified prospective volunteers who have the potential for unmonitored access to children. Our online form is posted here for downloading in PFD. All coaches must download the form, fill out, sign, and mail to the Doug Smith at his address below. All information received from the coaches is confidential and only the Commissioner will have access to it and only the Commissioner will receive the confidential information back from The Commonwealth of Massachusetts Criminal History Systems Board for review.
